The employment agreement (EA) form serves as a record-keeping tool that helps the employer manage the employee's information throughout their onboarding process. This section will explain everything you need to know about EA form templates from creating new templates to publishing templates, as well as how to manage country section form templates.

1. What Is EA Form Template?
EA form template enables users to configure their own EA form catered towards their needs. This is important because EA may differ by country or by industry of the company. This template is for Employment Agreement, which is a binding document signed by an employer and an employee.
2. Access to EA Form Templates
- Go to the sidebar and select
Onboardings - Click
EA Form Templates - The EA form templates will show up

There are 2 tabs on the EA form templates page:
- EA Form Templates Tab
- Country Section Form Templates Tab
3. Status of Templates
- Published: EA Form Template is successfully published
- Unpublished: Not yet published
- Changed: The EA form was published, but the information was changed
4. EA Form Templates Tab

4.1. What Is EA Form Templates Tab
EA Form Templates Tab is a section that displays all the EA form templates. You can create, edit, and archive the onboarding form templates from this tab.
4.2. Create New EA Form Templates
- Click
+ New EA Form Template - Set âEA Form Template Nameâ (e.g. Example EA Form)
- Click
Create Template - Update and Publish EA Form Template
- Select âMulti-page wizardâ for multiple pages
- Select âSingle page formâ for a single page
- This is for clients onboarding workers
- For more information about form components, users can refer to Form Elements section
- Click
Previewto view the form template - Click
Publishto finish editing - Once published, clients can share the form with a link
- The template will show up in EA Form Templates

4.1. Select âRegionâ
4.2. Select âForm Modeâ
4.3. Notification Email(s)
4.4. Drag and drop a form component from the sidebar


4.3. Edit EA Form Templates
- Click anywhere on the row of the template that you want to edit (e.g. Example EA Form)
- Click
Edit and publish - Edit the information as you want
- Click
Publish - The form will change accordingly

4.4. Filter EA Form Templates
You can choose desirable criteria to filter lists of templates that you want to display in the table.
- Click
Filters - Select criteria that you want to filter (e.g. Country)
- Set your criteria (e.g. Japan)
- The filtered values regarding your criteria will show up in the table

The filter can be set and edited from the bar above the table as well.
4.5. Sort EA Form Templates
The table can be sorted alphabetically and numerically in descending and ascending order.
- If data in the selected column is alphabetic data, it will be sorted alphabetically
- If data in the selected column is numerical data, it will be sorted numerically
- If data in the selected column is in date format, it will be sorted from newest or oldest
The table can be sorted by one column at a time.
- To sort based on that column, click on the head of a column until you get the desired sort

- The sorted table will show up
4.6. Archive and Unarchive EA Form Templates
To archive EA form templates:
- Click anywhere on the row of the template that you want to edit (e.g. Example EA Form)
- Click
Send to archive - The status of the template will change to âArchivedâ

To unarchive EA form templates:
- Click anywhere on the row of the template that you want to edit (e.g. Example EA Form)
- Click
Unarchive - The status of the template will change to âPublishedâ

4.7. Result When EA Form Is Filled
- Client receives the EA form submission in their email
- EA submission can be reviewed by clicking
Review Submission


- Worker information can be reviewed by clicking âReview Worker Informationâ


- Email sent to client assigned to onboard a worker
5. Country Section Form Templates Tab

5.1. What Are Country Section Form Templates
Country section form can be made for specific countries with different requirements. This form will be added as an extra page when filling out âInformation Request for EA Prepâ form. Clients can select country section form in the first page of âInformation Request for EA Prepâ as seen below:

5.2. Create New Country Section Form Templates
- Click
+ New Country Section Form Template - Select a country (e.g. Thailand)
- Click
Create - Update and Publish Country Section Form Template
- This is for clients onboarding workers
- Select âMulti-page wizardâ for multiple pages
- Select âSingle page formâ for a single page
- For more information about form components, user can refer to Form Elements section
- Click
Previewto view the form template - Click
Publishto finish editing - The template will show up in Country Section Form Templates
